How to Enroll

Here is what you will need to apply:

  • TennCare card and any other insurance cards for each child
  • Copy of child鈥檚 birth certificate (can be official copy or Mother鈥檚 Copy)
  • Proof of income, including:
    • At least four check stubs
    • W-2
    • Income tax forms
    • Child support or public assistance print out
    • SNAP (Food stamps)
    • SSI or Families First
    • Social Security Letter
    • Unemployment Letter
  • Driver鈥檚 License
  • Proof of Address
  • Name, address, and telephone number of at least three emergency contact persons
  • Immunization Records
